I hereby authorize repairs to be done along with the necessary material and hereby grant you and/or your employee’s permission to operate the vehicle stated herein described on the streets, highways, or elsewhere for the purpose of testing and/or inspection. An expressed mechanic’s lien is hereby acknowledged on below-stated vehicle to secure the amount of repairs thereto. OC Moto Haus will not be held responsible for loss or damage to vehicle or articles left in vehicle in case of theft, accident, or any other cause beyond control. All responsibility would be held by vehicle owner and/or vehicle owner’s insurance. All vehicles not picked up within 24 hours of completion or estimate submission will be subject to an $80.00 charge per day on a seven-day-week calendar and eventual disposal if abandoned for longer than thirty days at vehicle owner’s expense. OC Moto Haus is not to be held responsible for vehicles in their possession in any capacity. Labor rate is $160.00 per hour. Any and all diagnostic work/teardown is initially a $160.00 flat rate, additional diagnostics will require supplementary estimate authorization by the vehicle owner. This document and preceding verbiage represents a tear-down estimate/work order if any work is completed before a supplemental estimate is created. The signature below authorizes such and a copy of this form will be provided upon request.

NOTICE: WE DO NOT ACCEPT MOTORCYCLES THAT ARE DISASSEMBLED TO ANY DEGREE. CUSTOMER PROVIDED PARTS ARE NOT PERMITTED, UNDER ANY CIRCUMSTANCES. WE ARE NOT RESPONSIBLE FOR DAMAGE INCURRED TO ANY COMPONENT OF YOUR MOTORCYCLE DUE TO AGE OR PRE-EXISTING CONDITION OF THE PART OR COMPONENT. During the course of service, old or brittle parts may incur damage during the normal removal and/or repair process. We cannot anticipate nor be culpable for any such damage. We will bring these items to your attention but will not be liable for cost of replacement (parts or labor).
